The Insurance and Risk Management division of the Office of the City Clerk is responsible for three main areas:
  • claim administration
  • risk financing
  • risk control

How to submit a claim

Municipal Act, 2001 S.O. 2001, Section 44, 10 requires that,
 
A claim must be filed and submitted within 10 days of the incident's occurrence.
 
To submit a claim, please prepare and submit the claim report form.

Submitting a written notice
 
Alternatively, a written notice or a physical claim report form can be submitted by mail, fax or email.
 
If you choose to submit the claim using the claim report form, you may complete the PDF version of the claim report form which is found on the right-hand side of your screen under Resources.
 
If you choose to submit a written notice of the claim, it must be sent by registered mail to the City of Vaughan Clerk and must contain notice of the injury and/or damage incurred, including the date, time and location of the occurrence.
 
The written notice should include the following:
  • Contact information
    • name of claimant
    • mailing address
    • phone number
    • email address
    • the above information of the person submitting the claim if the above information of the person submitting the claim is different from the claimant
  • Date
    • date of occurrence
    • time of day
  • Description of occurrence
    • briefly describe the actions leading up to the loss or injury
  • Type of damage or injury
    • provide details of the damages or injury
  • Name of other parties involved
    • advise if there are other parties involved in this occurrence other than the City (i.e. contractor, developer)
  • Photography
    • provide photographs of the damages and photographs the location where the incident occurred

Next steps

If you submitted a claim online, you will immediately receive confirmation of your submission. Within five business days, you will receive an email notifying you of your claim handling status and further information.
 
If you submitted as a written notice or a claim report form, a written acknowledgemnt will be sent from the Risk Manage Division confirming receipt of the claim within two weeks. This acknowledgement will include a claim number; keep the number as a reference for future communication.
 
The Risk Management division will begin an investigation based on the information provided and may be in contact to request additional information. Due to the uniqueness of each claim, the timeline concerning the final decision of a claim will vary.

Service of Litigation Claims
During the City Hall facility closure, personal service of litigation claim documents is not available at the front counter of the Office of the City Clerk.  As an alternate, documents being served on The Corporation of the City of Vaughan may be emailed to clerks@vaughan.ca

Claims involving community hazards

If a hazard is noticed in the community, please contact the appropriate department listed below:
  • For parks- and forestry-related hazards, contact parksandforestryoperations@vaughan.ca.
  • For ice- and snow-related hazards on roadways, contact works@vaughan.ca.
  • For all other hazards, notify Access Vaughan at 905-832-2281.

Claims involving York Region or Ministry of Transportation

  • For claims involving regional roads and infrastructure, contact York Region at 1-877-464-9675.
  • For claims involving the provincial highways and infrastructure please contact the Ministry of Government and Consumer Services, Risk Management and Insurance Services Branch, at 416-314-3445.
  • For claims involving Highway 407 ETR, fill out their damage claim form which can be found at the bottom of this page.
To determine whether a road is municipal, regional, or provincial, please reference this York Region road identification map.
 
Note: a fraudulent claim is a criminal offence. All fraudulent claims will be prosecuted to the full extent of the law.
