The City of Vaughan has a Mandatory Vaccination Policy (PDF) in effect that applies to all employees, students and volunteers. City staff are required to receive two doses of a Health Canada-approved COVID-19 vaccine to ensure the best protection possible against COVID-19. 

Next steps:

  • Select the “Submit Proof of COVID-19 Vaccination” button below to visit the secure site
  • Select “New” on a desktop or “…” on a mobile device
  • Upload proof of vaccination
  • Fill in your information and press “Save” 

Then, you will receive an email confirming that your submission was received. 

This policy applies to all staff regardless of your work location. Requests for an exemption can be made. The City will comply with its human rights obligations, make exceptions for applicable medical conditions and accommodate employees legally entitled to accommodation. Here are the forms to request an accommodation: medical (PDF) or human rights – religion or creed (PDF).

All personal information and submissions are confidential. 



How to get your proof of vaccination: 

  • Download your vaccination receipts from Ontario’s COVID-19 vaccination system at covid19.ontariohealth.ca
  • Call the Provincial Vaccine Booking Line at 1-833-943-3900
  • Take a photo of the Ministry of Health receipt you received when you got your second vaccine
  • Download the email receipt you received the day of completing your second vaccination
Those who choose not to receive a complete vaccine series and do not have a valid exemption will be required to attend mandatory education on the benefits of vaccination. After that, they may be subject to disciplinary action, up to and including dismissal, if they do not get vaccinated. York Region Public Health continues to operate vaccine clinics in convenient locations across York Region. For details, please visit york.ca/COVID19Vaccine.
